ICD-10-PCS coding is different than anything inpatients coders have done in the past. The root operation character determines which table coders will use to build an ICD-10-PCS code.

However, physicians do not need to use root operation terms in their documentation. This means coders must determine the correct root operation by reading the operative report, determining the intent of the procedure, and selecting the root operation that represents that intent.
